Disclosure Slides

Congratulations on having your Oral and/or Poster accepted for presentation at the 5th Canadian Obesity Summit. As you prepare our presentation, please work on your Disclosure Slides, and keep in mind:

• Each presenter is requere to include a Disclosure slides for each presentation & poster - even if nothing to disclose.
• Information required (Click for the Disclosure Slides sample)
  • Memebr of an Advisory Board or equivalent with a commercial organization
  • Member of Speakers Bureau(s)
  • Received payment from a commercial organization (gifts of "in kind" compensations)
  • Received grant(s) or an honorarium from a commercial organization
  • Mitigating Potential Bias
• Presenter disclosure will be included for each presenter

This slides must be visually presented to the audience AND verbalized by the speaker.
